Welcome to PDYHA registration for the 2007 – 2008 season. We have made every attempt to make this process as simple and efficient for players / parents and our registrar / treasurer. Please follow the instructions below and take your time when filling out the required information entries. When you have finished the entire registration process you should have completed 4 items.
They include:
1. USA Hockey registration online
2. Pee Dee Youth Hockey Association Online Payment
3. Completed Player Agreement – download and print
4. Completed Parent Agreement – download and print

PART ONE: USA Hockey
Step One: Please click on the USA Hockey ONLINE REGISTRATION logo below which will take you to USA Hockey’s registration page for the 2007 – 2008 season. The instructions provided by USA Hockey are very simple yet detailed and will allow you to register your youth player for this season. All PDYHA players MUST register in order to participate this year and must be registered BEFORE the first on ice session. For players age 6 and younger this registration is free. For those players age 7 and older the fee is $38.00.
Step Two: After registering with USA Hockey you will receive a confirmation e-mail. Please forward this receipt to the league REGISTRAR’S e-mail address. You may also wish to print a copy for your own records.

PART TWO: Pee Dee Youth Hockey Association
Step One: Please complete the order form below which will take you to an “order” page which accepts Visa and Mastercard credit cards. After entering the customer information and credit card data finalize your “order”. Your “order” is the 2007 – 2008 league fee of $80.00.
Step Two: VeriSign, a trusted name in credit card processing, will send you a confirmation / receipt e-mail after finalizing your order. Please forward this e-mail confirmation to our TREASURER’S e-mail address. You may also wish to print a copy of this receipt as well for your records.

GENERAL OVERVIEW
Offline registration consists of downloading the player and parent agreement forms, completing and signing the forms and then bringing them to the first on ice session where they will be collected. Please click on the individual links below to download and print these two forms.
